Andy Lewis, the extreme athlete who helped bring slacklining to a global television audience during Madonna’s 2012 Super Bowl halftime show, died in a tandem BASE jumping accident in southeastern Utah. He was 39.

The accident occurred on June 14, 2026, in the remote Mineral Bottom area of Grand County, near Moab. Lewis was jumping with Danny Joe Kregle, 68, of Arizona, when both men suffered fatal injuries. The Grand County Sheriff’s Office confirmed that the two were participating in a tandem jump.

In a tandem BASE jump, a passenger is harnessed to an experienced jumper and both descend using a single parachute. Kregle had sought out Lewis’s company, BASE Jump Moab, for the experience, according to local reporting and statements from his family.

Lewis was one of the best-known personalities in the modern slacklining and BASE jumping world. He helped popularize tricklining — performing acrobatic tricks on a flexible line — and won four consecutive world championships in competitive slacklining between 2008 and 2011.

His breakthrough into mainstream popular culture came in February 2012, when he appeared alongside Madonna during the Super Bowl XLVI halftime show. Wearing a Roman-style costume, Lewis bounced, flipped and performed tricks on a slackline in front of one of the largest television audiences in the world.

Lewis later based much of his life and business around Moab’s desert cliffs. He operated BASE Jump Moab and became a prominent tandem guide, introducing clients to a sport known for far less margin for error than traditional skydiving.

Kregle’s family described him as a devoted father and grandfather, an accomplished businessman and someone who loved travel and adventure. His relatives said they wanted him remembered for the life he lived rather than only for the accident that ended it.

BASE jumping remains one of the world’s highest-risk aerial sports because jumps are made from fixed objects at much lower altitudes than conventional skydives, leaving little time to react if something goes wrong. AP reported that a widely used BASE-fatality database has recorded more than 500 deaths worldwide since 1981.

Public reporting has offered differing accounts about what may have gone wrong with the parachute, but the Grand County Sheriff’s Office did not release a final technical accident report in the sources reviewed by The Informant. The safest confirmed conclusion remains that Lewis and Kregle died during a tandem BASE jump at Mineral Bottom.

For the extreme-sports community, Lewis’s death removed one of the figures who most aggressively pushed slacklining and BASE jumping into mainstream visibility. His career was built around balancing extraordinary skill with extraordinary risk — the same tension that made him famous and ultimately defined the final chapter of his life.

Andy Lewis, the extreme athlete who helped bring slacklining to a global television audience during Madonna’s 2012 Super Bowl halftime show, died in a tandem BASE jumping accident in southeastern Utah.
